Financial Assistance: The Joysanne Sidimus Fund

The Joysanne Sidimus Fund is a financial assistance program which helps eligible professional artists, full-time students in, and teachers at, recognized post-secondary arts institutions (as determined by the Eligibility Checklist below) to pay for the fee-for-service treatments offered at The Al & Malka Green Artists’ Health Centre (Centre) by covering 75% of the cost, up to a maximum of $760 for each successful application.

This fund has been named in honour of Joysanne Sidimus, whose vision and passion were instrumental in the creation of the Artists’ Health Alliance.

Please note that this program is funded entirely through generous private donations and funding is subject to availability of funds. This financial assistance is only available as short term support and is not to be relied on for ongoing medical care and treatment.

To Apply:

Step 1:
Before applying OR re-applying for financial assistance you must have had an appointment with the Nurse Practitioner at The Al & Malka Green Artists’ Health Centre.
Applications will not be accepted until an assessment has been complete.
This appointment is free of charge and can be made by calling The Al & Malka Green Artists’ Health Centre at 416-603-5263.

Step 2:
Download and fill out the correct application form. If you are not sure which form to fill out, please call our office at 416-351-0239 or email admin@artistshealth.com.

Step 3:
Send your completed application and supporting documents (as outlined in the application form) to the Artists’ Health Alliance:

Applications are reviewed once a month. Upon committee review, you will be notified via email about the status of your application.

Below are the upcoming deadlines to submit an application for review for that month:

• February 24, 2016 at 11am
• March 29, 2016 at 11am
• April 25, 2016 at 11am
• May 30, 2016 at 11am
• June 27, 2016 at 11am

Applications received after each month’s deadline will be reviewed the following month.
