Becoming A Patient

Are you interested in becoming a client of The Al & Malka Green Artists’ Health Centre (Centre)?

Referrals are not required. If you self-identify as an artist, call us at 416-603-5263 to make an appointment with the Nurse Practitioner for an initial assessment. This hour-long appointment provides the opportunity to become acquainted with clinic services, discuss any of your concerns and explore treatment options available to you.

Prior to the appointment you will be asked to complete the Declaration of Professional Status as an Artist. To be accepted as a client of the Centre, artists must meet at least one of the requirements of the Declaration.

Cancellation Policy

All patients who miss appointments and/or cancel with less than 24 hours’ notice will be required to pay the full-fee for their appointment. If you need to reschedule or cancel an appointment please contact Rita at (416) 603-5263.
